Help on Course Certificate for students. Is it properly written?

Hi teachers,
I wonder if you could help me on this matter.
I always give my students a 'course certificate' after they have finished a course.
The thing is I would also like to write down the number of hours they have attended.
Is it correct this way? Any suggestions are more than welcome.
Thanks in advance

Punctuate as if the whole statement were a normal sentence:

This is to certify that
________________________

has completed an 80-hour basic level course in English as a Foreign Language

from ___________________ to _________________
