Help me solve this problem

Paragraph: Every test maker is different. Some worry about randomising their answers, while others never give it a thought. My aggregate data suggest, however, that you can bet on some widespread patterns even when going into a test cold.

I have get confused with the phrase "test cold". Could anybody please define it to me? Thanks a lot.
