Help me how to request for a Salary Increase

am kinda feel shy to ask salary increase to my boss even i believe am entitle to have increment. serving the company for three years i receive 1,000.00 increment and not that i have finish nearly to another two year. would like to request for increment ti inspire me and to help my family need specially my siblings schooling.

can you please repolish my letter in a way he might consider to give me rewards

I would like to request you to please review my performance and kindly consider my salary increment request. I am willing to do other task you may required in return.
