HELP! APOLOGY LETTER RE. GARDA VETTING

Hi! I really need help right now. This may be embarassing at my part but I made a very silly silly silly mistake in applying for Garda Vetting. I have ticked Yes in the question "Have you ever been convicted of an offence in the Republic of Ireland or elsewhere".
I was suppose to tick No, but for some weird,silly, odd reasons i ticked YES. I made a formal letter and im not sure if its formal enough. Pls help me to copy furnish it. Thanks.

I, Name, a recently employed care staff at name of nursing home, would like to apologise for the mistake I have made in applying the Garda Vetting Form, as I misread the contents. I have ticked the box "Yes" instead of ticking "No" to the Question "Have you ever been convicted of an offence in the Republic of Ireland or elsewhere". I hope you will reconsider my application.
