She has had a lot of cosmetic surgery in an attempt to disguise her age.
Is it talking about disguising her appearance? Like looking younger?

No. Disguising her age. She had the surgery so that she would look younger, and presumably more beautiful. But she didn't want to disguise her appearance. That would make her look like someone else, and her friends wouldn't know who she was anymore.
