The baby spits out some milk.
He has a sour smell cause he spits out.

The second sentence sounds a little bit weird and not natural.
How to say the sour smell appeared after he spits out milk.

By "half-digested" I gather that you mean the baby swallowed the milk and later brought it back up. "spit out" doesn't really convey this; you would use another term, such as "throw up", "be sick", "vomit".

For example, "The baby smells bad because he threw up."
