Having a difficile to something

For example, if somebody struggles for having Clostridium difficile,

does it mean that his/er immune system's having trouble combating with such type of superbug?

Or does the difficile a superbug itself? Because the sentence I saw went as

"The patients who take PPIs are more likely to have Clostridium difficile, a super dangerous superbug to eliminate."

The use of the pronoun here just confuses me.
