Have this listening related doubt?

Dear friends,I use to listen to podcasts on various subjects as a routine for improving my english skills.I have this following doubt:
While listening should I have to concentrate on those "is" " was" "were" "had been" "would" "could"(tenses) etc.. OR
should I concentrate as a whole thoughts to understand it.(I mean thought by thought and what was being said)
