When I say " I had a doctor say to me that...", do I mean "A doctor said to me that..."? Then why can't I just say that instead of saying " I had a doctor say to me that..."?

You can if you like. 'I had a doctor say' is just an elided form of 'I had the experience of a doctor saying to me'.
