Why do we Have on a 3rd person when the question starts with Does?
For example: Does she have a deductible?
or Does the patient has a deductible?

The auxiliary verb (do, does, did) assigns the tense and the person, after which we use the base form of the verb.
He has: does he have; did he have.

Another way of saying it is that you can only have one s-marker per verb phrase. It has to be on the first verb.

does have, does know, does see OK
does has, does knows, does sees Not OK.
do has do knows, do sees Not OK.
