The DOH report from 2009 states that “ All chemotherapy prescriptions should be professionally checked by a pharmacist who has undergone specialist training, demonstrated the appropriate competence, and is locally authorised or accredited for the task. A list of designated pharmacists should be kept in the pharmacy department. This apples to oral as well as parenteral therapy”

please explain to me the verb-tense form used in " who has undergone specialist training, demonstrated the appropriate competence, and is locally authorised or accredited for the task."
has undergone=present perfect
demonstrated-simple past

"has" is intended to govern both "undergone" and "demonstrated", i.e. both are intended as present perfect, but the construction is not strictly correct. This is correct:

... who has undergone specialist training and demonstrated the appropriate competence, and is locally authorised or accredited for the task.
