Please explain which one is correct:

1)Ben - Just wondering if your wife has decided to join the gym?

2) Ben - Just wondering if your wife decided to join the gym?

and what about this one?
Has she decided to join the gym?

Both. Both communicate the same thing. The information content is the same. It's just the way the speaker is thinking about it that's different.

In 1, the speaker is asking for an update of how things stand now.
In 2, the speaker wants to know what happened.
