Regardless of the number of people, we use 'have' when the sentence is made into a question of this kind.

Do they have ... / Does he have ...


You could make a question of the statement by changing intonation and adding a question mark though:

'He has a pen.'

'He has a pen?'

But, never 'Does he has a pen?'
