Has broken/broke

1) He has broken three of his bones recently.
2) He broke three of his bones recently.

Can we tell if he broke them on the same occasion or on different ones?
I think in '2' it is clear that the three bones were broken on the same occasion, but '1' is somewhat unclear.

3) He has had three bone fractures on three different occasions.
4) He had three bone fractures on three different occasions.

Can one tell if it was one fracture per occasion or three fractures per occasion?

Do these work:
5) On three different occasions, he had a bone fracture.
6) On three different occasions, he broke a bone.
