1) Could you tell me whether (A) & (D) are correct, please. If (D) wasn't correct, I want to know the reason.

2) Check these sentences, please.

1. He went to hospital for an operation.

2. He went into hospital for an operation.

3. He went in hospital for an operation.

4. He went in to hospital for an operation.

If all are correct, I want to understand the difference in meaning between them, please.

I would like to add one thing to Clive's answer.

In American English, "hospital" must have an article.

2. He went into the hospital for an operation.

Better: He was admitted to a hospital.
