1) Could you tell me whether (A) & (D) are correct, please. If (D) wasn't correct, I want to know the reason.

2) Check these sentences, please.

1. He went to hospital for an operation.

If all are correct, I want to understand the difference in meaning between them, please.

I would like to add one thing to Clive's answer.

In American English, "hospital" must have an article.

2. He went into the hospital for an operation.

Better: He was admitted to a hospital.
