H&S issues

I need to report these Health and Safety issues; Could you please tell me if the sentences are correct?

1) The main office door that gives access to the reception makes loud noise when closes (or when it closes?), disturbing who is/works near by.

2) The fire extinguisher in room X was on the window-sill, causing the risk of breaking the window if dropped by accident. 3) The extinguisher must be fixed on a suitable place and the applicable fire extinguisher sign displayed next to it.

4) No Fire Warden has been appointed by the site supervisor, since person X (the last site´s fire warden) has left the company.

5) Adequate number of fire wardens are to be appointed by the site supervisor and adequate training given.

I would appreciate if you could help me.

Mr. P. - in order to be truly commensurate with your name, should it not be "An adequete number of fire wardens IS ...", as 'number' is singular?

I must admit, I do prefer a plural verb, with 'a number of'.

As you can imagine, this causes much outrage at family get-togethers...

In that case, for #2: '...if it had been accidentally knocked over'.

PS: 'with regard to sentence #2' would be fine. You could also say 'As for sentence 2...', in this context.
