Had/Had Been?

Hi friends,Am newbie. I was wondering,when do we use Had and Had been? I don't know grammatically but i use it in sentences when i speak out.

So, I need to know that where do we use it? Because Topic- "Errors" gonna come to my exam and i hope EF Members would help me.

As an auxiliary verb to form past perfect simple (had + 3rd form of a verb as in The train had (already) left before we reached the station ) or past perfect continuous (had + been + the -ing form of a verb as in "I had been waiting for her for more than an hour when she came.
