What is wrong with his eyes?

1) I don't know. I think he had a LASIK operation go wrong.
2) I don't know. I think he had a LASIK operation go wrong on him.
3) I don't know. I think he had a LASIK operation on him go wrong.

Which of the cases 1-3 are correct?

They are not wrong, but the function of "on him" is different in those two sentences.

Could one use:

2) I think he had a LASIK operation go wrong on him.

if he was the surgeon?

Or if he was a person another member of the operating team? I don't think it could used in this case.
