Had it coming?

When someone dies in an accident or end up in any other tragic death, some people say "he had it coming"

What does that really mean?

So if someone died in an accident, and people say "he had it coming" does that mean he drives/drove badly? through carelessness?

Or it generally mean that the person is a bad guy?

It could be either or both or something else which makes the accident appropriate revenge for his behavior or attitude.
