Had had had

How is the following sentence even possible, friend of mine were corrected with this by their grammar teachers. She wrote:

"She had four children and is pregnant again" and was corrected with the following:

"She had had had had four children in the past and is now pregnant again."

The correction should be,

"She had had four children..."
