these are the excerpts for one of the drug histories I did in the morning.
Amoxicillin 250mg/5ml oral solution- 10 mls TDS (200mls)
issued by the GP on 26/10/17. the patient had had already
12 doses before admission (last dose on 30/10/2017)
is "had had already" grammatical and sound natural in the context?
the idea; the patient was issued an antibiotics prescription on 26/10/2017 for 1 week but the patient could only take 12 doses before coming to admission.

It is grammatical, but the post-verbal location of "already" strikes me as odd. It would be much better to put it between the auxiliary and the participle: the patient had already had 12 doses.
Further, the participle had could be replaced with take: the patient
