Hi guys,

I don't hear 'How do you do?' much any more in N. America.

But note that it's a formulaic greeting. It's not a question where an answer is expected, so you don't need to say 'Fine, thank you' or 'Not so well, I have a pain in my back'.

The standard response is simply to reply 'How do you do?'

ie

A: How do you do?

B: How do you do?
