How can I start a response letter?

How are you? I am very well or Things are going very well

How can I say that I am well? What is the usual way of saying this?

If this is to someone you know very well,

Hi John,

How are you? I'm fine, and hope you are too. Or I'm doing well. Or even Things are great with me.

You said this is a response letter - to whom are you responding? That will determine whether you use a more formal tone. The ones above are casual.
