Grammar - subjunctive

I am currently teaching an advanced class the sunjunctive in English. I have searched everywhere for rules on the subjunctive but have found nothing clear. Specifically I am looking for the rules regarding tense agreement (e.g. past subjunctive with conditionals/present subjunctive with present simple/past simple/future simple etc.). I would like to present to my class a table showing subjunctive/indicative tense agreement. Is this possible?
