[grammar question] who all?

Dear Users,Here's a question from a quiz on a book called The Giver. Who all has to take these pills?I've never seen a question that would begin with who and be followed by all. Can someone explain it to me? I think it's incorrect.Thanks

It's fine as an informal construction. It is not limited to questions. Here are some from COCA:

...you can tell who all went to the Yves Saint Laurent exhibition in Paris, and who didn't...
...So who all in your household has it, Robin?
...Chief, who all was home? What time of a day was it when the boy allegedly...
...He wondered what it was like and who all would be there to link up with.
...so this year I'm just going to sit back and see who all's going to be winning.
...And who all is going? Are you invited?
...Stone is full of witches. I can't tell you who all is a witch, but you would be surprised.

Notice that although 'all' suggests plurality, the verb chosen here is singular (though plural is also possible). That suggests that 'who all' is an idiomatic phrase. And there also exists 'what all' and 'where all' at least:

What all are you going to buy at the supermarket:
Where all did you visit in Kyoto?
