Grammar question

Hi fellows!
I would like to help me clarify sth.... I have a grammar quesion and the sentence is like this:
'I'm fed up with these exams!'
'How many ........?'
a. don't you have left
b. have you left
c. did you have left
d. do you have left

I know that the right answer is 'd' but the thing is that I am wondering why is not 'b' as well???
