[grammar] Have I messed up the tenses?

Dear All, 

Could you tell me where else I should've used past perfect and why? Would you be also able to give this email a general grammar & punctuation scan?

I've just checked my outbox and realized that the message I intended to send was never sent to you. I was wondering how it might have happened that the email wasn't sent and it must have been because of my VPN that had been on on my work computer. Sometimes the connection breaks and there's no notification. So while I thought the message had been sent, in fact it wasn't.
