Yes, both words are okay. But the meanings are different.

This broken chair is a problem.

This house has a problem. Perhaps its furnace does not work?

This toilet has not sanitation. (Does not work.)

This toilet has poor sanitation. (okay, I think, though not positive.)

This toilet is unsanitary. (This is more natural.)

This word is an idiom in the States only. (I am assuming that you meant that this word is an idiom in the United States only.)

John AkiThis word is idiomatical in State only.

This is not a proper sentence because "idiomatical" does not exist. Idiom is a noun.
