1. I want to know if my eye is ok.
2. I want to know is my eye ok.
3. I want to tell you a story about how it all began.
4. I want to tell you a story about how it all begun.

I agree. Also, I'd write either "OK" or "okay". Not "ok".

To elaborate: #2 can be made grammatically acceptable with a few punctuation marks.
I want to know: is my eye OK?
(Not the best way to do it, but it is grammatically acceptable)
