Hi. I'm trying to create a survey form for some purpose, but I'm not quite confident with my grammar. Can anyone help me to look up on it? Thanks a lot!

The main purpose of this survey is to gather your thoughts and opinions, as part of (name of society), in order to create a better society for you in the future! This survey should only take 5-10 minutes to complete. You may or may not choose to complete all the questions below. Be aware that the answers you provided will be shared among the candidates that are running the election for 2016/17. Once again, Thank you for taking part!

What is your favourite activities in xxx?
What are the reasons that usually caused you for not attending/unable to attend xxx's event?
What are you expecting from the new members of committee/xxx for the upcoming year?
What are you expecting to gain after graduate from (University)?

It is really difficult to make suggestions because the survey seems to have cross purposes. Is is the students in a university, the citizens of a country, members of a group as a society? With all the missing words (***), it is difficult to understand.

Hi AlpheccaStars, thanks a lot for your reply! Apparently you've had a hard time helping me in correcting my survey form. It was a survey for members of a society to gather their thoughts and opinions, and all the *** is actually the name of the society. Anyway, I've already roughly got an idea on how to write it now! Thanks again for your effort, it was sincerely appreciated!
