The 'third form' is what some writers label teh traditional past participle. It is used with HAVE to construct perfect forms, and with BE to construct passive forms.

The 3rd form this is a kind of irregular verb (go-V1, went-V2, gone-V3) used in the present and the past prefect tense. For example: verb (V) - have (has) +V3. Also you have to use it in the passive voice if the verb is irregular - to be (was or were, been) + V3. Something like this. May be I forget a little bit.
