A friend wanted to ask me what happened, but instead asked "what did happen?" I explained to him that it would be correct to ask "what happened". Which is correct?

'What did happen?' is possible for emphasis.

A: My mother got totally the wrong idea about what happened at camp. She thought that I had got into a fight with the football coach.
B. What did happen? / What actually happened? / What really happened?
