Can we write "on appointment" or only " by appointment" is acceptable?

Dr. Smith sees patients only by appointment. This means every patient needs an appointment. You can't say 'on appointment' here.

On appointment as the new President of the company, he immediately moved into his new office. This means 'as soon as he was appointed'. You can't say 'by appointment' here.

I've never seen anything other than 'by'. However, other prepositions would make perfectly good sense, especially 'with'.
