One student I have is having trouble with Past Simple Passive 'Was' and Present Perfect Passive 'Has Been'

For example The decision was easy to make or The decision had been easy to make
Another example There was an accident or There had been an accident
Does anyone know of a simple explanation of these tenses and an easy way to remember their use.
