Dear Madam,
I would like to inform you that last Sunday, 26 of January 2020, I came to school (Name of School ****) 1 hour and 14 minutes late, As I went to the Health Center to buy a refill of my medicine and for blood test. I took this chance as there were no students due to a Republic Holiday and the staff timing is 8 am. I'd like to apologize also for not informing you ahead of time as i didn't expect that it would take me long time to complete the blood test and I had to wait for the doctor's advice.
Hoping for your kind and favorable approval regarding this matter to refrain any deductions from my salary due to above mentioned reason.
