I want to ask
Have you ever gone to a Kabbalah centet?, Have you been to a Kabbalh center? Or Did you ever go to a Kabbalahcenter?

"Uncorrect" is incorrect. And I see nothing wrong with the question "Have you ever gone to a Kabbalah center?" Nor do I think that "has/have gone" necessarily means that the person has not returned.
