Honestly I don't think it means much of anything. People seem to use it whenever they need an appropriately stinging punctuation mark at the end of a story, so I don't know that a formal definition can be nailed down. It annoys me simply because the people who say it don't know what it means, either. And so the sad circle of ignorance continues.
