Given name or first name

Hi. Let's say there is a Korean who has to fill out a form that asks for his last and given names. Let's also say his full name is Minsu Kim, with "Kim" being his last name and "Min" being his first name and "su" being his middle name. His friends call him "Minsu." What should he write in the space that asks for his given name? Thank you in advance for your help.
