Gain Access

"He gained access to the database."
b)
Does it mean A) he looked at and/or modified the database, or that he gained the right/permission/opportunity to look at and/or modify the database?

I was confused by definitions 2 and 3 in this dictionary, and they give examples for each definition:

def 2 "They tried to gain illegal access to the company's network"
def 3 "Investigators are trying to gain/get access to his financial records."
