Given that it is an adverb, what does it modify? Doesn't it modify the noun "Truths", making it an adjective instead?

It gives a time frame for the sentence. Once = at one time.
It does not modify "truths". You can have bare-faced truths, or hard truths, etc. These would be adjectives.
