from which point can I use somethings is repetited 'yet again' ?

I am not sure when I can use 'yet again' expression. I know its basic meaning but not sure that I can use 'yet again' from second repetition something occurs or from third repetition something occurs. Can I use 'yet again' in both situation which means from second repetition something happens? thanks a lot ~ here English forum have so lovely and kind people around~
