Formal versions of "cleanliness nut"

Can anybody think of a formal expression to indicate somebody who's highly concerned about hygience?

And one more question,

What is a "antal-retentive" cleanliness nut? Sounds so dirty to me...

Some formal adjectives are: Fastidious, meticulous, or scrupulous concerning their personal hygiene

chivalry"adverse psychological consequences of toilet training",
During toilet training, children can be punished for making mistakes and being dirty. This might have life-long psychological consequences (at least, according to the Freudians) - such as guilt, fixation on self-control and being clean, etc.
