Formal letter to apply emergency leave

RE : EMERGENCY LEAVE ON 19.07.2010
I am writing this letter to inform that I taken an emergency leave on 19.07.2010, Monday due my uncle pass away on 17.07.2010. I have to rush went back to my hometown on 17.07.2010 and only arrived (KLIA) on 19.07.2010.
I am kindly asking you to consider this as my request for a leave of absence.
