Re: Accident report on Forklift FL 175

On 28th April 2010 at about 1600 hrs, a forklift beam suddenly broken during the container is transfer to the container truck for delivery and falls cause dents the container.

2.0 The resurfacing process
The forklift truck was returned to the surface the next day at 0900 hrs.

3.0 Technical report on the breakdown of FL 175
As agreed in the meeting on 7 May chaired by Manager (Operation), the forklift was sent out to Gen Sen Engineering workshop for inspection. The inspection was held on 10 May.

4.0 FL 175 maintenance record
All repair work corresponding had been done. Up until the accident occurred, the were no complaints or outstanding work to be done on this particular FL 175.

5.0 Recommendation
A comprehensive quotation by Gen Sen, cost of repair the beam is about RM 5,000 which their re-use the old beam and welding. For replace the new beam is RM 8,000 but their need to order from supplier and take 3 days to get the beam.
