Forgot to do this and forgot to do that

I had to either lock the main door or turn on the alarm or do both of those things. I forgot to do both.

Which of the following sentences can I use in such a context:
1) I both forgot to lock the main door and to turn on the alarm.
2) I forgot to lock the main door and to turn on the alarm.
3) I forgot to lock the main door or to turn on the alarm.
4) I forgot to lock the main door and I also forgot to turn on the alarm.

I don't think '3' works and '2' seems to imply that I had to do both things.

I would go with 3. But I would delete "to" before "turn."

I forgot to lock the main door or turn on the alarm.
