For example

Some patients who do not respond therapies of depression may simply have received inadequate treatment, having, for example,been prescribed a drug as a dosage too low to be effective or having been taken off a drug too soon.

Why is for example after having? Is sentence trying to imply prescribing a drug as a .. only as an example?Will this sentence be grammatically all right if I change red text to drug as a dosage that was low to be effective?

Is there any difference between following construction?

Having been prescribed vs Having a drug prescription

Having been prescribed vs when they have a drug prescription.

prescribed a drug at a dosage that was low to be effective won't work. You must retain too.

prescribed a drug at a dosage that was too low to be effective is fine, but wordier than the original.
