for a letter, when do we use Dear and To

Hi when I write a letter, When I call the receiver of the letter

I'm a bit confused about the difference between

Dear xxx

To xxx

So when do we use first one and the latter one?

Any significant differences?

and the latter one? Never. Unless you want to write letters like:
