1) Is it possible to write 5F for 5th floor, 4F for 4th floor, 3F or 3rd floor ?
2) Is it possible to just write 5 for 5th floor, 4 for 4th floor?

Both make sense in a context where it is clear that you are referring to floors. However, I'm not sure that the "F" designation is used much natively here in England, if at all. When I do a Google search, it seems that most of the examples of the "F" style are from Japan. Possibly it is ?? or how you call it? Personally I would just use numbers 1, 2, 3 etc.

When a slash separates multi-word phrases, it's better to put spaces around the slash. Thus, normally I would write "Lobby/Restaurants", but "Conference Rooms / Vending Machines". However, in a list where both patterns are combined, it may look presentationally better to use spaces throughout, even for single-word alternatives, thus "Lobby / Restaurants".

Consider whether you need to say "Fitness Gymnasium". It seems that "Gymnasium" will probably suffice.
