One of her spaghetti straps has fallen off so she needs to put it back on her shoulder.

One of her spaghetti straps has fallen off but she she's just putting it back on her shoulder.

One of her spaghetti straps has /fallen/fallen off her shoulder/, so she adjusts it/puts it back in place.

"Fallen off" by itself suggests that maybe it has actually detached itself from the dress, it which case it would need to be mended or sewn back on -- but I don't think tht's what you mean here.

"Fixes" is actually okay in either sense!

No. This is not what I mean. Is there a way to remove this ambiguity?

Try "one of her spaghetti straps has slipped/fallen off her shoulder."
