Any good dictionary will tell you that you can fill in a form or you can fill out a form. You can fill up a tank with gas/petrol.

You fill up a form when you write information in the spaces as required: You have to fill up so many forms when you take your car abroad.

Hi,

In my experience, 'fill up a form" suggests that you have used up all the available space and need more. eg

Receptionist: Please fill out this form.

Me (a few moments later): I've filled it up.

Receptionist: OK, just write on the back.

Fill in the form means to complete it according to the instructions, in the blank spaces which are provided.

Fill up means to fill it completely.
